As we research and find more information, let us share it.
In regards to sucession...the heir to the throne is the oldest son, who I believe is required to prove himself as an adventurer. If no son is in place (as with the most recent sucession of Queen Neverfire), the oldest daughter is named. The child has to prove themselves worthy of the crown. If they are not worthy, another relative will be brought in to take their place.
This might be the event that Flame is speaking of...
This situation, of a child being found unworthy, has only occurred once in Dragon's Bane history. King Neverfire V named his sons unworthy of sucession and brought in one of his wife's kinsman, who shared a distant relation to the Neverfire bloodline.

This seems ta be tha occasion that Flame is referring to. How long ago did this occur?
It seems treachery and perhaps more sorted desires was behind this action, not tha unworthiness of tha child.
This whole thing with Flame is a bloody civil war started by human ambition it seems.

I am still working on that piece of the puzzle. However, I do have additional information...
As the tradition is for the heir to prove themselves worthy of succession and able to rule before they ascend to the throne, many of them do not have children until late in life. This means that the ruling families tend to be very long lived for humans, sometimes going into the hundreds before they pass. A monarch has a long rule, sometimes sitting the throne for at least forty years, sometimes longer. *I wonder if they have some magical means of prolonging their lives. ~Aasha*
The current Queen, Crystal, is the eleventh Neverfire. If the "incident" that we are investigating occurred at the onset of the Neverfire reign, we are looking at at least 400 years, and that is an ancient grudge, for certain.
